What seems to be happening here is that the cotton has not been correctly formed, so when it is expanding it is not retaining it’s shaped and falling apart inside people, and breaking apart when they try to pull on the string. I would assume that many of these people who have had issues didn’t realize they had remnants of cotton still inside them, and only realized something was wrong when they started to feel unwell.
